What is POI Hormone Replacement Therapy?

The aim of POI HRT is to relieve symptoms such as hot flashes, prevent osteoporosis and cardiovascular disease, preserve genitourinary health and support psychological wellbeing.

The estrogen dose is 1.5-2 times the standard postmenopausal dose: oral estradiol 2 mg/day, transdermal patch 50-100 μg or conjugated estrogen 0.625-1.25 mg/day. The transdermal route is preferred for VTE risk.

In patients with a uterus, cyclic or continuous combined progesterone is added. Combined oral contraceptives are an alternative option, but bioidentical regimens are more physiologic. Follow-up is performed by symptoms, bone density and lipid profile.
