What is Phaeochromocytoma Crisis?

Phaeochromocytoma is a chromaffin-cell tumour arising from the adrenal medulla or extra-adrenal sympathetic ganglia; in 10-15% of cases it is malignant and 30-40% is associated with familial syndromes.

Crisis presents with severe hypertension, headache, palpitations, sweating, abdominal pain, anxiety, hyperglycaemia, end-organ damage and life-threatening complications.

Diagnosis requires plasma free metanephrines, 24-hour urine catecholamines, abdominal CT/MRI and 123-I MIBG scintigraphy. Definitive treatment is preoperative alpha-blockade followed by laparoscopic adrenalectomy.
