What is Pericardial Tamponade — Emergency Drainage?

Cardiac tamponade is a condition in which rapidly accumulating pericardial fluid compresses the heart and obstructs venous return.

Causes include malignancy, uraemia, viral pericarditis, post-cardiac surgery and aortic dissection.

The clinical picture is dominated by Beck's triad: hypotension, distended neck veins and muffled heart sounds; emergency echocardiography confirms the diagnosis.

Symptoms

Acute shortness of breath and chest tightness
Hypotension and pulsus paradoxus (>10 mmHg drop)
Distended jugular veins (raised JVP)
Muffled heart sounds and tachycardia
Cool extremities and clouding of consciousness
Anxiety and feeling of impending doom
Decreased urine output and pre-renal failure

Risk Factors

Active malignancy (lung, breast cancer, lymphoma)
Recent cardiac surgery or cardiac catheterisation
Chronic kidney disease and uraemic pericarditis
Tuberculous pericarditis and viral infection
Anticoagulant use
Aortic dissection or chest trauma
Autoimmune disease (lupus, rheumatoid arthritis)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Emergency presentation by 112 ambulance is mandatory in shortness of breath, hypotension and shock signs
  • Bedside echo confirms tamponade and pericardiocentesis is performed within minutes
  • Acute decompensation may turn into cardiac arrest; early diagnosis is life-saving
  • Recurrence is common; surgical pericardial window may be required
