What is Pediatric Septic Shock Emergency Management?

Pediatric septic shock is a state of inadequate tissue perfusion that develops in response to a suspected or confirmed infection, with mortality reaching 10-25%. Children present a different clinical picture than adults, and signs may be subtle.

The pediatric sepsis bundle includes the following critical steps within the first hour: blood culture, broad-spectrum antibiotics, IV fluid bolus (20 ml/kg crystalloid), vasoactive support (epinephrine, norepinephrine), source control and lactate monitoring.

Cold septic shock (hypovolemic phenotype) and warm shock (vasodilatory phenotype) require different approaches. Stress dose hydrocortisone is added in catecholamine-resistant cases, and ECMO support is considered in refractory shock.

Symptoms

Fever or hypothermia
Tachycardia and tachypnea
Capillary refill >3 seconds
Cold or warm extremities
Decreased urinary output
Disorientation and disturbed consciousness

Risk Factors

Premature birth and immune deficiency
Indwelling central catheter
Recent surgery or trauma
Underlying malignancy
Chronic disease (CKD, congenital heart disease)
Immunosuppressive therapy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Fever and altered general status in child
  • Cold extremities and decreased capillary refill
  • Decreased urinary output
  • Sleepiness and lethargy
  • Petechial rash (meningococcus)
  • Persistent high fever
