What is Osteoid Osteoma?

Osteoid osteoma is a benign bone tumor with a central nidus (active osteoid-producing tissue) surrounded by reactive bone sclerosis. The nidus is usually less than 1.5 cm and produces characteristic pain due to prostaglandin synthesis.

It is most common in young men aged 10 to 25 years and is generally located in the cortex of long bones such as the femur and tibia. Lesions in the spine can cause painful scoliosis.

The classic description is pain that worsens at night, is unrelated to activity, and shows a dramatic response to aspirin or NSAIDs. This clinical feature is highly valuable for diagnosis.

Symptoms

Dull pain that worsens at night
Dramatic response to aspirin or NSAIDs
Localized tenderness
Joint effusion in lesions near a joint
Painful scoliosis with spinal involvement
Muscle atrophy
Growth disturbances in children

Risk Factors

Male sex (2-4:1 ratio)
Age 10 to 25 years
Cortex of long bones
Genetic predisposition (rare)
Familial clustering
White race
Prior bone pathology

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For bone pain that worsens at night
  • For bone pain relieved by aspirin
  • For unexplained localized bone pain
  • If painful scoliosis is developing
  • If pain has lasted more than 3 weeks
