What is Oral Immunotherapy for Food Allergy in Children?

Oral immunotherapy (OIT) is a treatment for IgE-mediated food allergies (peanut, milk, egg, tree nuts) involving gradually increasing oral doses of the allergen under medical supervision to achieve desensitization, reducing the risk of severe reactions to accidental exposure.

The protocol typically includes initial dose escalation under medical supervision, build-up phase with home dosing, and long-term maintenance. Peanut OIT (e.g., Palforzia) is FDA-approved for ages 4-17. Sustained unresponsiveness (the ability to discontinue OIT) is variable.

OIT carries risks of allergic reactions including anaphylaxis, eosinophilic esophagitis, and quality-of-life impact. Patient selection, careful monitoring, and emergency action plans are essential. OIT shifts the threshold but does not cure the underlying allergy in most patients.
