What is Obstructive Shock?

Obstructive shock is a form of shock caused by a mechanical pathology that physically obstructs cardiac filling or blood flow through major vessels. The heart's intrinsic pump function is generally preserved.

The main causes include massive pulmonary embolism, cardiac tamponade, tension pneumothorax and severe aortic stenosis.

Diagnosis is usually made clinically and with bedside ultrasound (FAST, lung US, echocardiography); treatment is unsuccessful unless the cause is removed.

Symptoms

Sudden shortness of breath and chest pain
Hypotension and tachycardia
Jugular venous distension
Loss of unilateral breath sounds in tension pneumothorax
Distant heart sounds and pulsus paradoxus in tamponade
Syncope or pre-syncope

Risk Factors

Prolonged immobilization after surgery
Cancer, malignancy
Trauma and rib fractures
History of pericardial effusion
Pregnancy and oral contraceptive use
Previous thromboembolic events

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 immediately for sudden shortness of breath and chest pain
  • Unilateral lung silence after trauma requires urgent intervention
  • Hypotension with neck vein distension warrants attention
  • Patients with syncope episodes need urgent evaluation
