What is Obstructive Defecation Syndrome?

Obstructive defecation syndrome is a chronic defecation disorder characterized by inability to fully evacuate stool, sense of incomplete emptying, and excessive straining. Causes include rectocele, rectal intussusception, sigmoidocele, pelvic floor dyssynergia (anismus), and perineal descent syndrome.

It is more common in women; pelvic floor disorders and rectovaginal connective tissue weakness following vaginal delivery and hysterectomy contribute to its development. Defecography, dynamic pelvic MRI, and anorectal manometry are important for diagnosis.

Treatment begins with biofeedback and pelvic floor physiotherapy for functional causes. For anatomical disorders, surgical methods such as rectocele repair, STARR (Stapled Transanal Rectal Resection), and ventral rectopexy are used.

Symptoms

Sense of incomplete evacuation
Need for excessive straining
Need for digital manipulation
Pain during defecation
Chronic constipation
Pelvic pressure and fullness
Anal and perineal pain

Risk Factors

Multiparity and vaginal delivery
Hysterectomy
Menopause
Obesity
Habit of chronic straining
Collagen tissue disorders
