What is Nephrocalcinosis in Children?

Nephrocalcinosis is the deposition of calcium salts within the renal parenchyma. The most common pediatric causes are prematurity with furosemide use, distal renal tubular acidosis, hyperoxaluria, and medullary sponge kidney.

It is detected easily on ultrasound and may progress to chronic kidney disease in advanced stages. Treatment of the underlying cause can slow progression.

Symptoms

Often asymptomatic and detected incidentally
Recurrent urinary tract infections
Failure to thrive
Polyuria and polydipsia in renal tubular acidosis
Hematuria when nephrolithiasis coexists

Risk Factors

Prematurity with furosemide and corticosteroid exposure
Distal renal tubular acidosis
Primary hyperoxaluria
Hyperparathyroidism
Dent disease
Vitamin D intoxication

When to See a Doctor?

  • Detection of nephrocalcinosis on ultrasound
  • Recurrent kidney stones
  • Findings of metabolic acidosis or electrolyte disturbance
  • Failure to thrive accompanying renal findings
