What is Massive Pulmonary Embolism Emergency Management?

Massive pulmonary embolism is a state of hemodynamic instability and right ventricular failure that develops when a large clot, usually originating from deep vein thrombosis, occludes the main branches of the pulmonary artery.

It may present with shock, hypotension, and cardiac arrest. CT pulmonary angiography is the gold standard for diagnosis, but bedside echocardiography aids rapid decision-making in unstable patients.

Systemic thrombolysis (alteplase), catheter-based interventions, and surgical embolectomy are treatment options. Anticoagulation must be initiated in all patients.

Symptoms

Sudden-onset severe shortness of breath
Chest pain (pleuritic in character)
Hypotension and shock
Tachycardia and tachypnea
Syncope (fainting)
Cyanosis and jugular venous distention

Risk Factors

History of deep vein thrombosis
Prolonged immobilization
Recent surgery (especially orthopedic)
Cancer and chemotherapy
Oral contraceptive use
Thrombophilia (inherited clotting disorders)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 911 immediately for sudden shortness of breath, chest pain, and dizziness
  • Go to the emergency department when shortness of breath develops after leg swelling
  • Do not delay for fainting accompanied by breathing difficulty

Treatment Methods

Systemic thrombolysis (alteplase or tenecteplase)

Heparin anticoagulation (concurrently or thereafter)

Catheter-based thrombectomy or local thrombolysis

Surgical embolectomy (if thrombolysis is contraindicated)

Hemodynamic support (vasopressors, fluids) and intensive care
