Mallet Finger

An extensor tendon injury in which the fingertip cannot be held straight.

What is Mallet Finger?

Mallet finger is an injury that develops as a result of tendon avulsion at the insertion of the extensor tendon on the distal phalanx, or as a dorsal avulsion fracture of the distal phalanx. Active extension of the finger is lost.

According to Doyle's classification: Type I (closed tendon injury), Type II (laceration), Type III (deep laceration), Type IV (distal phalanx fracture). Fracture types involving more than 30% of the joint surface require surgery.

It usually occurs when a ball strikes the fingertip during ball sports, or as a result of strain while making a bed or putting on socks. If left untreated, it may progress to chronic swan neck deformity.

Symptoms

Drooping of the fingertip (flexion deformity)
Inability to actively extend the distal phalanx
Pain and swelling at the fingertip
Tenderness over the joint
Dorsal hematoma
Passive correction is possible
Crepitus when associated with a fracture

Risk Factors

Ball sports (basketball, volleyball)
Finger trauma
Sudden flexion at older ages
Occupational hand injuries
Making a bed
Osteoporosis (for avulsion fracture)
Tendon degeneration

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When the fingertip droops
  • Inability to straighten the finger after trauma
  • Significant pain in the distal phalanx
  • If there is an open injury
  • If a fracture is suspected
