What is Lightning Strike?

A lightning strike delivers a direct or indirect high-voltage discharge that can reach hundreds of millions of volts. Mechanisms include direct strike, side flash, contact, step voltage, and blast effect.

Major clinical effects are cardiopulmonary arrest, central nervous system dysfunction, keraunoparalysis, tympanic membrane rupture, and the fern-like skin pattern called Lichtenberg figures.

Triage differs from convention; people who appear alive have a high chance of survival, so arrested patients are prioritized ('reverse triage').

Symptoms

Sudden loss of consciousness and cardiopulmonary arrest
Transient paralysis and sensory loss (keraunoparalysis)
Tympanic membrane rupture and hearing loss
Lichtenberg figures (branching erythema)
Confusion and amnesia
Retinal burn and cataract (late)

Risk Factors

Standing outdoors during a storm
Sheltering at high points or under trees
Water sports and fishing
Golf, football, and other outdoor activities
Contact with conductive surfaces
Mountaineering and camping

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Every lightning strike case should be evaluated in the emergency department
  • Patients in cardiac arrest take priority
  • Hearing, vision, and neurologic examination should be performed
  • Evaluation should occur after transfer to a safe environment
