Levator Ani Defect

What is Levator Ani Defect?

The levator ani muscle complex is the main muscle group forming the pelvic floor. It consists of the puborectalis, pubococcygeus, and iliococcygeus parts. Avulsion of these muscles from the pubic bone during vaginal delivery is called a levator ani defect.

It can be evaluated with 3D/4D pelvic ultrasonography and MRI. It can be unilateral or bilateral. In the presence of bilateral defect, the risk of developing pelvic organ prolapse in the future increases 2-4 fold.

Treatment depends on symptom severity. Pelvic floor physiotherapy, pessary, and surgical approaches are used. Knowing the presence of the defect can affect the success of prolapse surgery and guide the choice of approach.

Sense of pelvic fullness
Loose vaginal opening
Stress urinary incontinence
Pelvic organ prolapse
Sexual dysfunction
Difficulty with defecation
Asymptomatic (some cases)

Risk Factors

Vaginal delivery (especially first delivery)
Macrosomia
Prolonged second stage of labor
Forceps application
Advanced maternal age
Prolonged forceful pushing
