What is Kyphosis (Hunchback)?

Kyphosis refers to the backward angulation of the thoracic spine exceeding the normal range of 20–45 degrees. It is classified as postural kyphosis (correctable), Scheuermann kyphosis (structural, occurring during adolescence) and pathologic kyphosis (osteoporosis, tumor, infection).

Scheuermann kyphosis is defined by wedge-shaped deformation of more than 5 degrees in at least three adjacent vertebrae and is seen during adolescence. Pain and cosmetic concerns are the primary complaints.

Osteoporotic vertebral fractures are an important cause of secondary kyphosis in adults. Advanced kyphosis can adversely affect respiratory capacity, cardiac function and quality of life.

Symptoms

Visible hunchback in the spine
Back pain and fatigue
Pain that increases with forward bending
Difficulty breathing (in advanced cases)
Leg pain or numbness (with associated spinal stenosis)
Body image concerns and self-esteem issues

Risk Factors

Osteoporosis and vertebral compression fractures
Rapid growth during adolescence
Long-term forward-bent working posture
Neuromuscular diseases
Flattening after spinal surgery
Scheuermann disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • In case of visible curvature or postural deformity
  • When back pain is accompanied by walking difficulty
  • With rapid increase in curvature during adolescence
  • If respiratory distress is present

Treatment Methods

01
Postural kyphosis: physical therapy, postural exercises, strengthening
02
Scheuermann: bracing (Cobb angle 45–75°), exercise
03
Surgery: severe structural kyphosis or neurological compression
04
Osteoporotic kyphosis: osteoporosis treatment, vertebroplasty
05
Respiratory physiotherapy
06
Long-term postural follow-up
