What is Kyphosis (Hunchback)?

The normal kyphosis angle of the thoracic spine is 20-45 degrees. An angle exceeding 45-50 degrees is evaluated as pathological kyphosis. There are different types: postural kyphosis (usually in adolescence, correctable), Scheuermann kyphosis (structural, in young adults), and osteoporotic kyphosis (due to compression fractures in the elderly).

Scheuermann kyphosis occurs with wedge-shaped deformation of thoracic vertebral bodies during the growth period and often ranges between 45-75 degrees.

In the elderly, cumulative osteoporotic vertebral compression fractures lead to kyphosis and height shortening.

Symptoms

Rounded back or hunchback appearance
Pain in the middle back
Fatigue increasing with prolonged standing
Shoulders falling forward
Shortness of breath in advanced cases
Neck and low back pain (compensatory)

Risk Factors

Osteoporosis (vertebral compression fractures)
Scheuermann disease
Prolonged sitting in a bent position (desktop, phone)
Muscular dystrophy and neuromuscular diseases
Ankylosing spondylitis
History of radiation therapy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If the kyphosis angle is progressively increasing
  • If back pain is affecting daily life
  • If shortness of breath or difficulty swallowing is developing
  • If numbness or weakness in the legs has begun
  • If osteoporotic compression fracture is suspected

Treatment Methods

01
Posture correction exercises and physical therapy
02
Brace therapy (Scheuermann, during growth period)
03
Anti-inflammatory and pain-relieving drugs
04
Vertebroplasty or kyphoplasty (for osteoporotic fractures)
05
Posterior spinal instrumentation and fusion (severe, progressive)
06
Osteoporosis treatment and fall prevention
