What is Management of the Poor Responder in IVF?

Poor response in IVF is a clinical problem associated with insufficient oocyte retrieval and low cumulative live birth rates. The POSEIDON classification considers age, ovarian reserve markers, and the response to previous cycles.

Treatment strategies include microflare, antagonist protocol, luteal phase stimulation (dual stim), high-dose gonadotropin, LH support (HMG), androgen and DHEA priming, coenzyme Q10, and growth hormone support.

Specialized laboratory practices try to improve success rates with IVM (in vitro maturation), rescue ICSI, euploid embryo selection by PGT-A, and cumulative embryo banking.

Symptoms

Low AMH level
Low antral follicle count
Fewer than 4 oocytes in previous cycles
Advanced maternal age
History of long-term failed IVF
Reduced serum estradiol response

Ovarian surgery (endometrioma, oophorectomy)
