Iliotibial Band Syndrome (Runner's Knee)

Pain on the outer side of the knee caused by friction of the iliotibial band over the lateral femoral epicondyle.

What is Iliotibial Band Syndrome (Runner's Knee)?

The iliotibial band is a thick fibrous fascial structure formed by the merging of tensor fascia lata and gluteus maximus muscles on the lateral side of the hip; it inserts on the Gerdy tubercle on the outer side of the knee. When the knee is flexed at 30 degrees, the band rubs over the lateral femoral epicondyle.

When this friction is repeated during long-distance running, downhill activities, and faulty training programs, it leads to inflammation and pain. It is a common sports injury affecting approximately 12% of runners.

Hip abductor weakness, leg-length discrepancy, foot overpronation, and running on a sloped surface are important contributing factors.

Symptoms

Burning, sharp pain on the outer side of the knee
Pain that begins consistently at a certain distance during running
Pain worsening when going down stairs
Tenderness over the lateral femoral epicondyle
Resolution of pain after stopping running
Rarely, knee swelling

Risk Factors

Long-distance running
Sudden increase in training load
Running on sloped or hard surfaces
Hip abductor muscle weakness
Leg-length asymmetry
Overpronation

  • If lateral knee pain interrupts running at 2–3 km
  • If there is no response to conservative treatment for more than 6 weeks
  • Chronic pain affecting daily walking
