What is Hypoglycemia Emergency Management?

Hypoglycemia is the drop of blood glucose level below 70 mg/dL. It is common in people using insulin or oral antidiabetic drugs due to skipping meals, excessive exercise, or excessive medication.

Since the brain depends on glucose, severe hypoglycemia can result in confusion, seizures, and coma. Rapid blood sugar elevation is critical.

Symptoms

Tremor, sweating, and palpitations
Dizziness and weakness
Excessive hunger sensation
Pallor and cold-clammy skin
Difficulty concentrating and irritability
Loss of consciousness and seizures in severe cases

Risk Factors

Insulin or sulfonylurea use
Skipping meals or insufficient carbohydrate intake
Unexpected intense exercise
Alcohol consumption
Kidney or liver failure
Insulinoma (rare)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call emergency immediately if loss of consciousness or seizure develops
  • Go to emergency if no improvement in 15 minutes after giving sugar
  • Emergency evaluation is essential for someone never having had hypoglycemia before
  • In recurrent hypoglycemia, endocrinology follow-up is required for medication dose adjustment

Treatment Methods

15 g rapid carbohydrate to conscious person (3-4 sugars, 150 ml fruit juice)

Wait 15 minutes, give again if still low (15-15 rule)

IM glucagon (1 mg) or IV 50% dextrose to unconscious person

Complex carbohydrate-containing meal after sugar normalizes

Review of insulin/medication regimen in frequent hypoglycemia
