Hip Replacement Revision Surgery

What is Hip Replacement Revision Surgery?

Revision total hip arthroplasty involves removal of all or part of a previously implanted hip prosthesis (femoral stem, head, acetabular cup, or liner) and replacement with new components, often supplemented with bone graft, augments, or specialized implants for bone loss and instability.

The most common indications are aseptic loosening, polyethylene wear with osteolysis, periprosthetic joint infection, recurrent dislocation, periprosthetic fracture, implant fracture or modular junction failure, and adverse reaction to metal debris (in modular and metal-on-metal designs).

Modern revision surgery may use jumbo cups, structural allograft, tantalum augments, dual-mobility constructs, constrained liners, modular femoral stems, megaprostheses, and 3D-printed custom implants. Two-stage exchange is the gold standard for most chronic infections.

Symptoms

Persistent or new pain in the hip, groin, thigh, or buttock after a previous hip replacement
Mechanical symptoms: clicking, popping, or sensation of giving way
Recurrent dislocations of the prosthesis
Limp, leg-length discrepancy, decreased walking distance
Signs of infection: fever, sinus tract, persistent wound drainage, redness, warmth
Visible deformity or shortening after a fall (suggests periprosthetic fracture)

Risk Factors

Time since primary surgery (loosening risk increases after 10–15 years)
Younger, more active patient (higher wear)
Obesity, smoking, uncontrolled diabetes
Repeated minor or major trauma
Previous infection, immunosuppression
Suboptimal component positioning at primary surgery
Use of historic metal-on-metal or modular taper designs

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • New or worsening hip pain that is mechanical, persistent, or interferes with sleep
  • Sudden inability to bear weight after a fall in someone with a prior replacement
  • Recurrent prosthesis dislocation
  • Wound drainage or systemic signs of infection
  • Routine surveillance imaging shows lucency, osteolysis, or implant migration

Treatment Methods

Comprehensive workup: clinical exam, X-ray, CT, possibly MARS-MRI, infection markers (CRP, ESR), joint aspiration with cultures and synovial fluid biomarkers

Single-stage revision for non-infected loosening or wear

Two-stage revision (resection arthroplasty + antibiotic spacer, then reimplantation) for chronic infection

DAIR (debridement, antibiotics, implant retention) for early acute infection in well-fixed implants

Bone-loss reconstruction: structural allograft, trabecular metal augments, custom triflange or 3D-printed cup

Dual-mobility or constrained liner for recurrent instability

Multidisciplinary approach: orthopedic surgeon, infectious disease, anesthesia, rehabilitation

Postoperative protected weight-bearing, hip precautions, prolonged antibiotic therapy when indicated, structured rehabilitation
