What is Management of Hereditary Hemochromatosis in Pregnancy?

Pregnancy creates a natural 'phlebotomy' effect due to plasma volume expansion and fetal-placental iron requirements; this may reduce iron load in women with hereditary hemochromatosis. However, in some patients, history of liver fibrosis and cardiomyopathy poses additional risks during pregnancy.

Diagnosis is made with transferrin saturation, ferritin, HFE gene analysis, and hepatic MRI T2* when needed. Routine phlebotomy or chelation therapy is generally not required in pregnancy; case-based evaluation is made in advanced load with very high ferritin.

Maternal management includes liver function tests, echocardiography, and glycemic monitoring. In the postpartum period, the phlebotomy protocol is reorganized, and HFE genotype analysis of the newborn may be planned.

Family history of hemochromatosis
Previous liver fibrosis
Past cardiomyopathy
Confirmed iron overload
