What is Heel Spur (Calcaneal Spur)?

Heel spur (calcaneal spur) is a bony prominence formed in response to repetitive stretching forces at the point where the plantar fascia attaches to the heel bone. It appears as a characteristic spur-like extension on radiography.

The presence of a heel spur does not always cause pain; it is frequently found incidentally in asymptomatic people. The main source of pain is usually concomitant plantar fasciitis or soft tissue inflammation.

The treatment approach is largely the same as plantar fasciitis treatment. The presence of a spur alone does not constitute a surgical indication; surgery may be considered in cases unresponsive to 6-12 months of conservative treatment.

Symptoms

Heel pain that intensifies on the first morning step
Tenderness with direct pressure under the heel
Pain increasing with prolonged walking
Worsening with walking barefoot on hard surfaces
Pain in the back of the heel (with Haglund deformity)
Sometimes swelling and local warmth

Risk Factors

History of plantar fasciitis
Flat foot or excessive pronation
Obesity
Prolonged standing occupations
Use of unsupportive shoes
Running and excessively loaded sports activity

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If heel pain lasts longer than 2 weeks
  • If you have serious difficulty walking
  • If painkillers are not responding
  • If there is swelling and redness in the heel
  • If there is no improvement despite 6 months of conservative treatment
