Hallux Rigidus

What is Hallux Rigidus?

Hallux rigidus is a condition in which dorsiflexion of the first MTP joint progressively decreases due to wear of the articular cartilage and formation of dorsal bony prominences (osteophytes) typical of joint osteoarthritis.

It differs from hallux valgus: the toe does not deviate medially, but the joint cannot move in a straight plane. If conservative treatment fails, several surgical options are available.

It is slightly more common in men than in women and usually becomes symptomatic between ages 30-60.

Symptoms

Pain and swelling at the first MTP joint
Pain that worsens during push-off in walking
Difficulty running and climbing stairs
A bony prominence on the top of the toe
Inability to lift the great toe upward or restricted lift
Relief with stiff-soled shoes

Risk Factors

A long first metatarsal or elevated first metatarsal head
History of trauma to the great toe
Excessive pronation
Family history of hallux rigidus
Intense sports activity

  • Progressive limitation of motion in the great toe
  • Pain interfering with daily walking
  • If unresponsive to conservative treatment, for surgical evaluation
