What is Gynecologic Emergencies?

Gynecologic emergencies cover life-threatening conditions including ectopic pregnancy, ovarian torsion, pelvic inflammatory disease, placental abruption and postpartum hemorrhage. Most require rapid surgical evaluation.

Ectopic pregnancy is an important cause of acute abdominal pain in reproductive-age women and may present with hypovolemic shock if rupture occurs. Early ultrasound and beta-hCG measurement are critical for diagnosis.

In every reproductive-age woman, certain investigations should not be performed before pregnancy is excluded; this approach is standard in emergency department practice.

Symptoms

Lower abdominal pain
Abnormal vaginal bleeding
Pain along with missed period
Dizziness, syncope
Shoulder pain (sign of intra-abdominal bleeding)
Tachycardia, hypotension
Fever and vaginal discharge (in infection)

Risk Factors

History of prior ectopic pregnancy
Previous pelvic infection and surgery
Assisted reproductive treatment
Sexually transmitted diseases
Pregnancy and delivery complications
Smoking

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call emergency services for sudden lower abdominal pain and dizziness
  • Go to the emergency department immediately for bleeding inconsistent with pregnancy
  • High fever and pelvic pain require infection evaluation
  • Pregnancy must be excluded in every reproductive-age woman
