Gonorrhea

A sexually transmitted infection caused by Neisseria gonorrhoeae.

What is Gonorrhea?

Gonorrhea is a major public health problem worldwide with rising antibiotic resistance. In women it causes cervicitis and PID; in men it causes urethritis and epididymitis. The throat and rectum may be involved in either sex.

About half of women are asymptomatic, leading to delayed diagnosis and ascending spread. Gonococcal PID may result in chronic pelvic pain, ectopic pregnancy, and infertility.

Due to antibiotic resistance, first-line therapy has changed; today the only accepted option is intramuscular ceftriaxone. Oral cephalosporins are no longer recommended.

Symptoms

Yellow-green vaginal discharge
Burning during urination
Intermenstrual or post-coital bleeding
Pelvic pain and fever (PID signs)
Sore throat (pharyngeal involvement)
Rectal discharge or pain (rectal involvement)

Risk Factors

Sexually active women under 25
Multiple sex partners
Not using barrier methods
Prior history of gonorrhea
HIV positivity
Commercial sex work

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When abnormal discharge or pelvic pain begins
  • If a sexual partner is diagnosed with gonorrhea
  • Annual screening recommended in sexually active women under 25
  • Urgent evaluation when fever and pelvic pain coexist

Treatment Methods

Ceftriaxone 500 mg IM single dose (≥150 kg: 1 g)

Adding azithromycin or doxycycline for chlamydia co-treatment

Concurrent partner treatment is mandatory
04
Test-of-cure (NAAT) within 1-2 weeks after treatment

Culture and susceptibility testing if resistance is suspected
06
Annual gonorrhea/chlamydia screening (under 25 or in risk groups)
