What is Approach to Geriatric Trauma?

Older trauma patients should be evaluated differently because of osteoporosis, sarcopenia, fragile skin and reduced cardiopulmonary reserve. Even a simple fall may result in serious injuries such as hip fracture, intracranial hemorrhage or rib fracture.

Anticoagulant, antiplatelet and polypharmacy use increase bleeding risk and warrant rapid imaging. Drugs such as beta blockers may also mask tachycardia and hide shock.

The approach includes age-adjusted ABCDE assessment, early imaging, fracture immobilization, pain management, delirium prevention and coordination with a multidisciplinary geriatric care team.

Symptoms

Hip pain and inability to walk after a fall
Altered consciousness and subdural hematoma in head trauma
Rib fracture and respiratory failure
Soft tissue injuries
Occult shock (normal pulse, cool extremities)
Delirium and confusion

Risk Factors

History of falls and balance disorders
Osteoporosis and sarcopenia
Polypharmacy (antihypertensives, sedatives)
Decreased vision and hearing
Fall hazards at home (rugs, bathrooms)
Dementia and cognitive impairment

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Every elderly patient who has fallen needs hospital evaluation
  • Cranial CT should be performed with a low threshold in patients on anticoagulants
  • Hip pain in an elderly patient requires X-ray and MRI if negative
  • Pain control is important to prevent delirium
