What is HSV (Genital Herpes)?

Genital herpes is a chronic viral infection caused by HSV-2 (the main agent) and HSV-1, transmitted through sexual contact. The primary infection usually presents with severe symptoms, while recurrences are milder and shorter.

After the initial infection, the virus remains latent in the sacral nerve ganglia and may reactivate due to immune weakening, stress, or hormonal changes, causing symptoms or asymptomatic viral shedding.

Active genital herpes in pregnancy can be transmitted to the newborn during delivery (neonatal herpes) and may cause severe neurological consequences. Therefore, careful management during pregnancy is required.

Symptoms

Painful vesicles and ulcers in the genital area
Burning, itching, and tenderness
Swollen and painful lymph nodes in the groin
Fever, headache, and malaise in primary infection
Burning or pain when urinating
Prodrome before recurrences: tingling, burning sensation

Risk Factors

Sexual contact with a virus-carrying partner
Not using condoms
Multiple sexual partners
Immunosuppression
Female sex (transmission is easier from male to female)
Mucosal damage or other STI

If you experience any of the following symptoms, seek medical attention promptly:

  • When painful blisters or sores appear in the genital area
  • If there is fever accompanied by painful genital lesions
  • When having a new or active herpes outbreak during pregnancy
  • If outbreaks are becoming more frequent or severe

Treatment Methods

Antiviral: acyclovir, valacyclovir, or famciclovir

Primary outbreak: 7-10 days of antiviral therapy

Recurrent: early-period episodic treatment (2-5 days)

Daily suppressive therapy: in frequent recurrences or HIV-positive cases

Suppressive therapy from week 36 in pregnancy

Avoid sexual intercourse during active lesions
