Fetal Growth Restriction (IUGR)

What is Fetal Growth Restriction (IUGR)?

IUGR is a condition in which fetal weight falls below the 10th percentile of expected values, with pathological growth restriction. It may be symmetric (early onset, all measurements small) or asymmetric (late onset, head sparing).

Causes may be maternal (hypertension, diabetes, smoking, poor nutrition), placental (placental insufficiency, previa, abruption), or fetal (chromosomal anomalies, infection, multiple gestation). Placental insufficiency is the most common cause.

Babies with IUGR have higher perinatal mortality, neonatal hypoglycemia, hypothermia, polycythemia, and a long-term risk of metabolic syndrome. Early diagnosis and close fetal well-being monitoring are life-saving.

Symptoms

Fundal height behind the gestational week
Estimated fetal weight <10th percentile on ultrasound
Oligohydramnios (decreased amniotic fluid)
Reduced perception of fetal movements
Reversed/absent end-diastolic flow in the umbilical artery
Abnormal Doppler findings

Risk Factors

Gestational or chronic hypertension
Preeclampsia
Smoking, alcohol, and substance use
Poor maternal nutrition and underweight
Advanced maternal age
Previous IUGR history and multiple pregnancy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When fundal height is found to be small
  • When decreased fetal movements are felt
  • In known maternal hypertension or preeclampsia
  • If smoking continues during pregnancy
  • When growth arrest is observed on ultrasound follow-up
