What is Femoroacetabular Impingement (FAI)?

Femoroacetabular impingement (FAI) is an important cause of hip pain and early osteoarthritis in young adults. It has three subtypes: cam type (femoral side), pincer type (acetabular side), and mixed type.

Cam type involves an aspherical femoral head-neck junction; pincer type involves over-coverage of the acetabulum (coxa profunda, retroversion). Repetitive mechanical contact damages the labrum and cartilage.

Diagnosis uses alpha angle (>55°) and CE angle. MR arthrography is the gold standard for evaluating the labrum and cartilage. Untreated, FAI progresses to hip osteoarthritis.

Symptoms

Deep pain in the groin
Pain that worsens with sitting
Pain in hip flexion and internal rotation
Stiffness after prolonged sitting
C-sign (patient grips the greater trochanter)
Activity restriction
Catching sensation in the hip

Risk Factors

Young, active adulthood
Sports such as soccer, ice hockey, ballet
Cam or pincer anatomical variants
High-impact activities during adolescence
Family history
Prior SCFE
Prior Legg-Calvé-Perthes disease
