What is Evaluation of Abdominal Pain in Children?

Abdominal pain is one of the most common pediatric complaints; it may be acute (hours to days) or chronic / recurrent (more than 2 months) and requires age-specific evaluation.

Common causes vary by age — infants: intussusception, malrotation, hernia incarceration; preschool: gastroenteritis, urinary tract infection, constipation, appendicitis; adolescents: appendicitis, ovarian or testicular torsion, mesenteric lymphadenitis, functional dyspepsia, and inflammatory bowel disease.

Red flags include night-waking pain, weight loss, blood in stool, jaundice, joint involvement, perianal disease, and a family history of inflammatory bowel disease — they prompt prompt laboratory evaluation, imaging, and referral.

Symptoms

Periumbilical pain migrating to the right lower quadrant (suspect appendicitis)
Episodic, severe colicky pain in an infant with vomiting and bloody 'currant jelly' stools (suspect intussusception)
Recurrent abdominal pain with weight loss, growth failure, or anemia
Pain accompanied by blood in stool or perianal lesions
Severe testicular or pelvic pain in adolescents
Pain awakening the child from sleep or persistent night pain

Risk Factors

Recent gastroenteritis or food intake suggesting infection
Family history of inflammatory bowel disease, celiac disease, or familial Mediterranean fever
Constipation, anxiety, and school absenteeism
Adolescent age and onset of menarche
Trauma or recent abdominal surgery

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Severe localized abdominal pain, persistent vomiting, or signs of dehydration
  • Bloody stools, jaundice, or unexplained weight loss
  • Pain with high fever, lethargy, or rigid abdomen — emergency evaluation
  • Recurrent pain with night awakening or growth failure

Treatment Methods

Age-specific history and full examination including abdominal, perianal, and genital evaluation

Laboratory testing such as complete blood count, CRP, urinalysis, liver and kidney function, lipase, stool studies, celiac serology, and fecal calprotectin when indicated

Imaging with abdominal ultrasound first-line and CT or MRI when surgical pathology is suspected

Treatment of identified causes — appendectomy, antibiotics, fluid resuscitation, dietary modifications, or referral for IBD management

For functional abdominal pain, multimodal management including reassurance, dietary advice, behavioral therapy, and follow-up
