What is Endometritis?

Endometritis is an inflammatory process directed at the endometrium lining the uterine mucosa. The acute form usually develops as a polymicrobial infection after childbirth (especially cesarean section) or intrauterine intervention (curettage, hysteroscopy, IUD insertion).

Chronic endometritis follows a more silent course and has been associated with implantation failure, recurrent miscarriage, and infertility. It is recognized by hysteroscopic biopsy and specific staining (CD138).

Postpartum endometritis manifests with high fever, uterine tenderness, and foul-smelling discharge. Controlling risk factors and prophylactic antibiotics (before cesarean section) significantly reduce the incidence.

Symptoms

High fever (above 38°C)
Lower abdominal pain and uterine tenderness
Foul-smelling or purulent vaginal discharge
Excessive bleeding during the postpartum period
Tachycardia and weakness
Mild pelvic pain and recurrent IVF failure in the chronic form

Risk Factors

Cesarean delivery
Prolonged labor and premature membrane rupture
Group B streptococcus colonization
Anemia and obesity
Intrauterine interventions (curettage, hysteroscopy)
History of sexually transmitted infection

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When fever begins after childbirth or intervention
  • When abdominal pain and foul-smelling discharge are seen together
  • In a history of recurrent implantation failure or miscarriage
  • When excessive or prolonged bleeding occurs in the postpartum period

Treatment Methods

Acute endometritis: IV antibiotics (gentamicin + clindamycin combination)

Mild cases: outpatient treatment with oral antibiotics

After childbirth: IV treatment continued until fever resolves

Curettage may be necessary in retained placenta

Chronic endometritis: doxycycline ± metronidazole for 14 days

Prevention with prophylactic antibiotics before cesarean section
