What is Emergency Management of Eclampsia in Pregnancy?

Eclampsia is a tonic-clonic seizure or coma developing on a background of preeclampsia. It can be seen from the 20th week of pregnancy or in the first 6 weeks after delivery.

Magnesium sulfate is the first choice in seizure prophylaxis and treatment. It reduces maternal mortality and cerebrovascular complications.

Once the diagnosis is made, the timing of delivery is planned considering gestational age and maternal stabilization.

Symptoms

Severe headache
Visual changes (scotoma, blurring)
Right upper quadrant pain
Severe hypertension (>160/110)
Proteinuria
Generalized tonic-clonic seizure
Altered consciousness
Oliguria

Risk Factors

Chronic hypertension
Diabetes
Renal disease
First pregnancy
Multiple pregnancy
Obesity
Abnormal uterine artery Doppler
History of preeclampsia

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Go to emergency for severe headache and visual changes after week 20
  • Follow-up for high blood pressure, edema and proteinuria
  • Call 112 in sudden pain and seizure
  • Symptoms can continue within the first 6 weeks after delivery

MgSO4 4-6 g loading over 20 minutes

1-2 g/hour maintenance infusion

Additional 2 g MgSO4 in recurrent seizure
