Dupuytren Contracture

What is Dupuytren Contracture?

Dupuytren contracture is a benign fibroproliferative disease of the palmar fascia of the hand. Palmar nodules develop initially; over time, cords form and flexion contracture of the fingers occurs.

It most often affects the 4th and 5th fingers. It is more common in men of European origin over 50 and is also called Viking disease. There is a clear familial predisposition.

The disease passes through three phases: proliferative (nodule), involutional (cord formation), and residual (contracture). Coexistence with similar diseases of the plantar fascia (Ledderhose) and penis (Peyronie) can be seen.

Symptoms

Firm nodules in the palm
Skin pitting and dimpling on the palm
Difficulty straightening the fingers
Flexion deformity of the 4th and 5th fingers
Difficulty washing hands or removing items from a pocket
Pain (in the early phase)
Progressive functional loss

Risk Factors

Men over the age of 50
Northern European descent
Family history
Diabetes mellitus
Alcohol and smoking
Epilepsy and antiepileptic drugs
Manual work and trauma

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When a palmar nodule or hardness is noticed
  • Difficulty straightening the fingers
  • Positive table test (hand cannot lay flat on a table)
  • Daily activities are impaired
  • Contracture exceeds 30 degrees
