What is Deep Vein Thrombosis Treatment in Pregnancy?

Pregnancy is a hypercoagulable state that increases the risk of venous thromboembolism (VTE) 5-10 fold. Deep vein thrombosis (DVT) is more common in the left leg (due to compression of the left iliac vein by the uterus).

D-dimer is not reliable in diagnosis as it is physiologically elevated in pregnancy; Doppler ultrasound is preferred. Low molecular weight heparin (LMWH) is used in treatment; warfarin is teratogenic and contraindicated in pregnancy.

Symptoms

Unilateral leg swelling
Calf pain and tenderness
Redness and warmth of the leg
Shortness of breath (suspect pulmonary embolism)
Chest pain (in PE)

Risk Factors

Thrombophilia (Factor V Leiden, prothrombin mutation)
Previous VTE history
Immobilization and bedrest
Obesity (BMI>30)
Cesarean delivery
Preeclampsia

  • When unilateral leg swelling develops
  • Shortness of breath and chest pain - EMERGENCY (suspect PE)
  • Prophylaxis assessment in pregnant women with DVT risk factors
  • Leg swelling in postpartum period

Treatment Methods

LMWH (enoxaparin) treatment dose (1 mg/kg twice daily SC)

Stop LMWH 24 hours before delivery

Continue postpartum anticoagulation for at least 6 weeks
