What is De Quervain's Tenosynovitis?

De Quervain's tenosynovitis is the inflammation and narrowing of the first dorsal compartment sheath that surrounds the abductor pollicis longus (APL) and extensor pollicis brevis (EPB) tendons on the radial (thumb) side of the wrist.

Triggers include carrying babies, social media use, playing musical instruments, and repetitive grasping or squeezing motions. Because it is common in new mothers, it is also called 'baby wrist'.

Diagnosis is made with the Finkelstein test: with the thumb folded into the palm, the wrist is bent toward the ulnar side, reproducing the typical pain.

Symptoms

Pain and swelling on the thumb side of the wrist
Pain that worsens with thumb movement
Difficulty with grasping and squeezing
Sharp pain with gripping motions
Tenderness over the radial styloid
Sometimes crepitus with movement

Risk Factors

New mothers (baby carrying and breastfeeding posture)
Repetitive thumb and hand movements
Excessive smartphone and tablet use
Pregnancy and the postpartum period
Rheumatoid arthritis
Musicians and manual workers

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If wrist pain lasts longer than 2 weeks
  • If you cannot perform daily activities
  • If the Finkelstein test is positive
  • If pain does not respond to analgesics
  • If pain prevents newborn care
