What is Clavicle Fracture Subtypes?

Clavicle fractures are divided into three groups according to the Allman classification: Type I middle third (80%), Type II lateral third (15%), Type III medial third (5%). The Neer classification subdivides lateral fractures further.

Middle third fractures usually heal with conservative treatment; however, surgical indication increases with shortening greater than 2 cm, 100% displacement, or comminuted fractures. Lateral third fractures may be unstable due to coracoclavicular ligament injury.

In surgical treatment, plate-and-screw fixation is the gold standard; intramedullary fixation is an alternative. Complications include nonunion, malunion, and neurovascular injury.

Symptoms

Swelling and tenderness over the clavicular region
Visible deformity (clavicular protrusion)
Pain on shoulder movements
Ecchymosis and hematoma
Need to support the arm
Crepitus
Skin tenting in open fractures

Risk Factors

High-energy trauma (motorcycle, bicycle)
Falling on the shoulder
Falling on an outstretched arm
Sports activities (rugby, football)
Osteoporosis
Traffic accidents
Birth trauma (newborn)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Pain and deformity after a fall on the shoulder
  • Inability to move the arm
  • Fractures with broken skin integrity
  • Neurovascular symptoms
  • Respiratory distress (suspected pneumothorax)
