What is Chondromalacia Patellae?

Chondromalacia patellae is degeneration of the articular cartilage on the posterior surface of the patella, characterized by softening, fibrillation, and erosion. It can be the underlying cause of patellofemoral syndrome.

Outerbridge classification defines four stages: Stage I (softening), Stage II (surface irregularity under 5 mm), Stage III (irregularity over 5 mm), Stage IV (erosions extending into subchondral bone).

Common in young female athletes. Advanced cases may progress to patellofemoral osteoarthritis.

Symptoms

Anterior knee pain that worsens with activity
Crepitus when bending the knee
Pain on stairs and ramps
Pain when rising from prolonged sitting
Knee swelling (advanced stage)
Tenderness on patellar compression

Risk Factors

Female sex and wide Q-angle
Repetitive knee flexion movements
Patellar maltracking
Quadriceps weakness
Excess body weight
Previous knee trauma

  • Chronic anterior knee pain with crepitus
  • Pain unresponsive to 6 weeks of conservative treatment
  • For staging with MRI or arthroscopy
