What is Chlamydial Infection?

Chlamydia trachomatis is the most commonly reported bacterial sexually transmitted infection worldwide. As an obligate intracellular pathogen, it replicates only in living cells and is asymptomatic in most cases.

Untreated chlamydial cervicitis can progress within weeks to months to endometritis, salpingitis, and PID. Each PID episode causes permanent tubal damage; ectopic pregnancy and infertility risks accumulate over time.

Mother-to-infant transmission during delivery may cause neonatal conjunctivitis and pneumonia. Therefore chlamydia screening is routinely recommended in the first trimester of pregnancy.

Symptoms

Increased cervicovaginal discharge
Post-coital bleeding
Mild pelvic or lower abdominal pain
Burning during urination
70-80% of cases are completely asymptomatic
Cervical friability on examination (contact bleeding)

Risk Factors

Sexually active women under 25
New or multiple sex partners
Prior history of chlamydia or gonorrhea
Not using barrier methods
Socioeconomic risk factors
HIV positivity

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • For annual screening in women under 25
  • For STI screening after a new sexual partner
  • If a partner has been diagnosed with chlamydia
  • At the first-trimester routine prenatal visit

Treatment Methods

Azithromycin 1 g single oral dose or doxycycline 100 mg twice daily for 7 days

In pregnancy: azithromycin or amoxicillin

Concurrent partner treatment is mandatory

Avoidance of intercourse until treatment is completed

Test-of-cure (NAAT) 3-4 weeks later

Retesting in 3 months recommended for recurrent chlamydia
