Cesarean Scar Ectopic Pregnancy

What is Cesarean Scar Ectopic Pregnancy?

CSP is diagnosed by transvaginal ultrasound when the gestational sac is seen in the lower uterine segment within the cesarean niche. If the pregnancy progresses, there is a high risk of placenta accreta spectrum and uterine rupture.

Treatment options include systemic or local methotrexate, suction curettage, hysteroscopic resection, laparoscopic wedge resection, and uterine artery embolization. Fertility-sparing approaches are generally preferred.

In subsequent pregnancies, recurrence of CSP, repair of the scar defect, and careful follow-up should be planned. Hysterectomy is considered only in selected cases of massive bleeding or treatment failure.

Symptoms

Vaginal bleeding in early pregnancy
Low pelvic pain
Abnormal beta-hCG rise
Scar localization on ultrasound
Sac visualized within the scar niche
Presence of fetal cardiac activity

Risk Factors

History of more than one cesarean section
Scar defect (niche)
IVF pregnancy
History of dilatation and curettage
Late diagnosis in the first trimester
Adenomyosis
