Cervicitis

What is Cervicitis?

Cervicitis is inflammation of the columnar epithelial cells lining the endocervix. The term mucopurulent cervicitis describes the yellow-green discharge from the cervical canal. Neisseria gonorrhoeae and Chlamydia trachomatis are the most important agents; Mycoplasma genitalium and herpes simplex virus may also be implicated.

It can be symptomatic or asymptomatic. Asymptomatic cases are detected through routine STI (sexually transmitted infection) screening using cervical swab cultures or NAAT testing.

Untreated cervicitis can spread upward, leading to endometritis, salpingitis, and PID. In pregnancy, chlamydial cervicitis increases the risk of preterm birth and neonatal infection.

Symptoms

Yellowish or greenish vaginal discharge
Bleeding after intercourse or during examination
Pelvic pain or pressure sensation
Burning during urination
Most cases are asymptomatic
Cervical tenderness (on examination)

Risk Factors

Multiple sexual partners
Not using barrier contraception
Previous STI history
Young age
New sexual partner
After intrauterine procedures

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When abnormal vaginal discharge or odor is noticed
  • When bleeding occurs after intercourse
  • If a sexual partner has been diagnosed with an STI
  • When routine STI screening is due

Treatment Methods

For gonorrhea: ceftriaxone IM single dose

For chlamydia: azithromycin single dose or doxycycline for 7 days

Avoid intercourse until treatment is complete

Concurrent treatment and notification of the partner

Test-of-cure at the end of treatment

Annual STI screening recommended
