Carpal Tunnel Syndrome

What is Carpal Tunnel Syndrome?

Carpal tunnel syndrome results from compression of the median nerve beneath the transverse carpal ligament (flexor retinaculum) at the wrist.

The carpal tunnel contains nine flexor tendons and the median nerve; any space-occupying or edematous process can increase intra-tunnel pressure.

Symptoms involve the median nerve distribution (thumb, index, middle, radial half of ring finger), with thenar atrophy in advanced cases.

Diagnosis uses clinical signs (Tinel, Phalen, durkan compression tests), nerve conduction studies (prolonged distal motor and sensory latency), and ultrasound showing nerve enlargement.

Symptoms

Numbness and tingling in thumb, index, middle, and radial ring finger
Nocturnal paresthesias that wake patient from sleep
Pain radiating to forearm or shoulder
Weakness with pinch grip and dropping objects
Thenar muscle atrophy in chronic cases
Positive Tinel sign (tapping median nerve) and Phalen test

Risk Factors

Female sex (3:1 ratio)
Age 40–60 years
Pregnancy (often resolves after delivery)
Diabetes mellitus, hypothyroidism, rheumatoid arthritis
Obesity
Repetitive wrist flexion/extension, vibration exposure, prolonged computer use

  • Persistent nocturnal hand numbness and tingling
  • Thenar muscle atrophy or persistent weakness
  • Failure of conservative treatment after 4–6 weeks

Treatment Methods

Neutral wrist splinting, especially at night

Activity modification and ergonomic adjustments

NSAIDs for associated tenosynovitis

Local corticosteroid injection (temporary benefit in most patients)

Carpal tunnel release (open or endoscopic) for persistent symptoms, thenar atrophy, or severe electrodiagnostic changes

Treatment of underlying systemic conditions (diabetes, hypothyroidism)
