What is Cardiogenic Shock?

Cardiogenic shock is the clinical picture in which tissue perfusion becomes inadequate as a result of impaired cardiac pump function. The most common cause is acute myocardial infarction.

It is characterized by hypotension, oliguria, altered consciousness and cold periphery. Mortality is around 40-60%.

Early revascularization (primary PCI), mechanical circulatory support and intensive care approach improve outcomes.

Symptoms

Severe hypotension (systolic <90 mmHg)
Cold, clammy and pale skin
Tachycardia or bradycardia
Oliguria (<30 ml/hour urine)
Confusion, lethargy
Signs of pulmonary edema and dyspnea

Risk Factors

Acute myocardial infarction
Cardiac valve disease and dysfunction
Acute myocarditis
Rhythm disorders
Pulmonary embolism
Cardiotoxic drug exposure

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 if chest pain is accompanied by weakness and faintness
  • Make an emergency call immediately for cold sweating, dyspnea and confusion
  • Rapid presentation is mandatory in sudden deterioration of a patient with prior heart attack
  • Difficulty standing up and decreased urine output require documentation

Treatment Methods

01
ABC stabilization, oxygen and monitoring
02
ECG and bedside echocardiography
03
Inotrope support (dobutamine, noradrenaline)
04
Primary percutaneous coronary intervention (PCI)
05
Mechanical circulatory support (IABP, ECMO, Impella)
06
Multidisciplinary follow-up in the intensive care unit
