Brodie Abscess

Subacute or chronic localized intramedullary bone abscess, classically in metaphyseal long bones.

What is Brodie Abscess?

Brodie abscess is a subacute or chronic form of hematogenous osteomyelitis that produces a localized abscess within bone, most often in the metaphysis of long bones in children and young adults.

Staphylococcus aureus is the most common pathogen, and presentation is insidious with mild pain and minimal systemic symptoms.

Diagnosis is supported by characteristic imaging features including a well-defined metaphyseal lytic lesion with sclerotic rim, and treatment combines targeted antibiotics with surgical curettage when needed.

Symptoms

Insidious bony pain often nocturnal
Mild localized swelling
Slight tenderness on palpation
Limited limb function
Mild fever or absence of systemic symptoms
Pain relieved by aspirin or NSAIDs
Long history before diagnosis

Risk Factors

Childhood and adolescence
Recent skin or soft tissue infection
Immunosuppression
Sickle cell disease
Diabetes mellitus
Previous untreated bacteremia
Penetrating injury near bone

  • Persistent unexplained bone pain
  • Pain that worsens at night
  • Localized swelling without obvious trauma
  • Failure to thrive in a child with limb pain
  • Persistent low-grade fever

  • Worsening despite oral antibiotics
