Breast Engorgement (Milk Stasis)

What is Breast Engorgement (Milk Stasis)?

Breast engorgement is breast fullness arising from excessive milk accumulation and vascular-lymphatic congestion that begins after lactation starts (usually days 3–5). Almost every breastfeeding mother experiences it to a mild-to-moderate degree.

Severe engorgement makes it difficult for the baby to latch onto the nipple; breastfeeding becomes painful and the mother may consider 'giving up'. The nipple flattens and the areola becomes tense.

The cornerstone of treatment is frequent and effective breastfeeding, supplemented by pumping when needed to empty the breasts. Supportive measures such as ice, leaves (cabbage) and massage are helpful.

Symptoms

Bilateral painful, tense and swollen breasts
Flattening of the nipple
Mild warmth and redness
Difficulty for the baby to latch
Mild fever (below 38°C)
Axillary pain and tenderness

Risk Factors

Delayed initiation of breastfeeding
Insufficient feeding frequency
Improper latch by the baby
First delivery
Late skin-to-skin contact after cesarean
Use of tight bras

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When engorgement lasts longer than 48 hours
  • If high fever or local redness develops
  • If the baby cannot latch onto the nipple
  • If there are nipple cracks
  • When lactation counseling is needed
