Benzodiazepine Poisoning

What is Benzodiazepine Poisoning?

Benzodiazepines (diazepam, alprazolam, midazolam, lorazepam, etc.) bind to GABA-A receptors, enhancing inhibitory neurotransmission. They are widely used for anxiety, insomnia, and seizures.

Mortality is generally low with benzodiazepine overdose alone; however, respiratory depression and death risk increase when combined with alcohol, opioids, or tricyclic antidepressants. Multiple drug ingestion must be investigated.

Flumazenil is a benzodiazepine receptor antagonist; it can cause seizures in chronic users and mixed poisonings, so it is used in selected cases. Treatment is generally supportive.

Symptoms

Sedation, drowsiness, and lethargy
Dysarthria and ataxia
Hypotonia and decreased deep tendon reflexes
Hypotension and mild bradycardia
Respiratory depression (especially with co-administered drugs)
Loss of consciousness (in severe cases)

Risk Factors

Concurrent alcohol or opioid use
Drug accumulation in elderly
Liver dysfunction
Use of long-acting preparations
Suicide attempt
Tolerance loss in chronic users with sudden high doses

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Call 112 in any suspected overdose
  • Respiratory depression requires emergency intervention
  • Observation period should be extended in mixed drug ingestion
  • Elderly patients should be monitored for at least 12 hours
