What is Bacterial Vaginosis?

Bacterial vaginosis (BV) is a dysbiosis of the vaginal microbiome in which Lactobacillus species are replaced by anaerobic organisms (Gardnerella vaginalis, Atopobium vaginae, Prevotella).

BV is not considered a sexually transmitted infection, but sexual activity, douching, and new or multiple partners are risk factors.

In pregnancy, BV increases risk of preterm labour, preterm premature rupture of membranes, and low birth weight; it also increases susceptibility to HIV and other STIs.

Diagnosis uses Amsel criteria (homogeneous grey discharge, pH >4.5, positive whiff test, clue cells on microscopy) or a Nugent score; up to half of cases are asymptomatic.

Symptoms

Thin, homogeneous grey-white vaginal discharge
Fishy odour, especially after intercourse
Mild vulvovaginal itching or burning
Discharge that coats the vaginal walls
Vaginal pH greater than 4.5 (normal 3.8-4.5)
No symptoms in about half of cases

Risk Factors

New or multiple sexual partners
Vaginal douching
Smoking
Intrauterine device use
Oestrogen deficiency
Recent antibiotic use

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If abnormal odour or discharge is noticed
  • For any vaginal symptoms during pregnancy
  • With recurrent infections
  • Before gynaecological procedures (hysteroscopy, dilatation and curettage)

Treatment Methods

Oral metronidazole 500 mg twice daily for 7 days, or metronidazole vaginal gel for 5 days

Clindamycin vaginal cream for 7 days as an alternative

Treatment is essential during pregnancy with safe antibiotics

Routine treatment of sexual partners is not recommended

Avoid vaginal douching and irritant soaps

Boric acid pessaries or lactobacillus probiotics may reduce recurrences
