Asherman Syndrome

What is Asherman Syndrome?

Asherman syndrome arises when fibrotic bands form within the uterine cavity following trauma to the endometrium. The most common cause is aggressive curettage performed to evacuate retained products of conception; infections and uterine surgery may also contribute.

Clinical features include amenorrhoea, hypomenorrhoea, cyclic pelvic pain from haematometra, infertility and recurrent miscarriage. Diagnosis uses hysterosalpingography, sonohysterography and hysteroscopy — the latter is the gold standard for both diagnosis and treatment.

Treatment relies on hysteroscopic adhesiolysis followed by measures to prevent re-adhesion such as an intrauterine balloon, high-dose oestrogen or hyaluronic acid gel.

Symptoms

Amenorrhoea or markedly reduced menstrual flow
Cyclic pelvic pain
Infertility
Recurrent miscarriage
Placental abnormalities in pregnancy
Dysmenorrhoea

Risk Factors

Previous dilatation and curettage
Postpartum-haemorrhage curettage
Septic abortion
Tuberculous endometritis
Myomectomy and septum resection
Repeated intrauterine surgery

When to See a Doctor?

  • When menstruation stops or becomes very light after curettage
  • Cyclic pelvic pain plus infertility
  • Recurrent miscarriage
  • After uterine surgery if symptoms develop
