What is Anomaly (Detailed) Ultrasound 18-22 Weeks?

The detailed (Level II) ultrasound is an examination performed by a perinatology-trained physician that systematically evaluates all of the fetus's organ systems (CNS, heart, lungs, GI, GU, skeleton). It typically lasts 30-45 minutes.

This examination can detect structural disorders such as central nervous system anomalies, cardiac defects, cleft lip-palate, renal anomalies, and skeletal dysplasias at a rate of 70-95%. Additionally, the amniotic fluid, placenta, and cord are also evaluated.

If a suspicious finding is detected during the examination, fetal echocardiography, fetal MRI, or amniocentesis may be recommended. Early diagnosis is critical for the delivery plan and organization of neonatal care.

Symptoms

Fetal cardiac defects (VSD, AVSD, tetralogy of Fallot)
CNS anomalies (ventriculomegaly, spina bifida)
GI anomalies (gastroschisis, omphalocele)
Renal anomalies (hydronephrosis, renal agenesis)
Skeletal dysplasias and limb defects
Soft markers: echogenic bowel, nuchal fold

Risk Factors

Advanced maternal age
Family history of congenital malformation
High risk on first-trimester screening
Maternal disease such as diabetes or epilepsy
Use of teratogenic drugs
IVF pregnancy

When to See a Doctor?

  • For the planned anomaly screening between weeks 18-22
  • To the perinatology council when an abnormal finding is detected
  • If fetal echocardiography is needed (suspected cardiac anomaly)
