What is Surgery for Advanced Uterine Prolapse?

The POP-Q system grades uterine prolapse from stage 0 to IV. In stage III the most distal point lies 1 cm beyond the hymen; in stage IV there is total eversion. Neglected cases may develop decubitus ulceration, hydroureteronephrosis, and infection.

Surgical options include abdominal sacrocolpopexy (ASC), laparoscopic/robotic SC, transvaginal sacrospinous ligament fixation (SSLF), uterosacral ligament suspension (USLS), and obliterative colpocleisis.

Apical support repair is combined with anterior-posterior compartment repairs, midurethral sling, and hysterectomy when indicated. FDA warnings on mesh use and complications must be considered.
