What is Advanced Evaluation Algorithm of Adnexal Mass?

Adnexal mass includes ovarian or fallopian tube-derived ovarian cysts, endometriomas, serous/mucinous tumors and malignancies. In evaluation, clinical examination, transvaginal ultrasound, CA-125, HE4 and ROMA/ADNEX calculations are used.

The IOTA (International Ovarian Tumor Analysis) group's 'simple rules' and the ADNEX model have become standard in estimating the probability of malignancy. Solid component, papillary projection, ascites and vascularization are evaluated on ultrasonography.

The management algorithm is shaped according to benign features, size (<3-5 cm vs large), menopausal status, tumor marker levels and patient risk factors. Suspicious cases are referred to gynecologic oncology.

Family history of ovarian or breast cancer
BRCA1/2 or Lynch syndrome
Advanced age
Long-term hormone exposure
Endometriosis
Nulliparity
