Adenomyosis

A painful gynaecologic disease in which endometrial tissue invades the uterine wall.

What is Adenomyosis?

In adenomyosis the endometrial glands and stroma penetrate the myometrium where they undergo cyclical bleeding and inflammation. The uterus enlarges, becomes firm and adopts a globular contour.

It is most often seen in parous women aged 40-50, but it can also occur in young, nulliparous women. Coexistence with endometriosis is common.

Diagnosis is suggested by pelvic ultrasound or MRI showing a thickened, heterogeneous junctional zone; histopathology after hysterectomy provides the definitive diagnosis.

Symptoms

Severe dysmenorrhoea
Heavy menstrual bleeding (menorrhagia)
Chronic pelvic pain
Dyspareunia
Symmetrically enlarged, tender uterus
Anaemia from chronic blood loss

Risk Factors

Multiparity
Previous uterine surgery (caesarean, myomectomy, curettage)
Tamoxifen use
Coexisting endometriosis or fibroids
Reproductive age (40-50 years)
Genetic predisposition
